Job summary
Telos Health Systems in Wilmington, NC seeks an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to lead internal medicine services in Skilled Nursing and Assisted Living settings. You will collaborate with an interdisciplinary team, perform comprehensive health assessments, and manage patient care under established protocols.
The role offers 2 nonconsecutive daytime weekdays, active PMHNP certification with DEA, and a supportive environment focused on quality of life for
Qualifications
- Certification as a PMHNP with active license and DEA registration.
- Minimum one year experience in a Skilled Nursing setting (preferred).
- Ability to build rapport with patients and families; thrive in a fast-paced healthcare environment.
- Strong written and verbal communication and leadership within a clinical team.
Responsibilities
- Lead internal medicine services as SNF-ist and collaborate with interdisciplinary team.
- Provide comprehensive health assessments and prescribe treatments under protocol.
- Perform physical examinations and manage patient care plans.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and facilities.
