Job summary

Telos Health Systems, Inc. is seeking an experienced Advanced Practice Provider (NP/PA) to serve as the in-house SNF-ist, delivering general medicine care to geriatric residents in SNFs and ALFs around Owenton, KY.

You will manage care for short- and long-term residents, perform comprehensive exams, and collaborate with medical directors to optimize resident outcomes. Sign-on bonus $10,000 with W-2 employment and flexible scheduling.

Qualifications

• Active, unencumbered KY APRN/PA-C license.
• Current DEA registration.
• Master’s degree from accredited NP/PA program.
• Strong communication and clinical leadership skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Serve as the facility’s primary internal medicine provider (SNF-ist).
• Manage care for short-term rehab and long-term care residents.
• Perform comprehensive health assessments, physical exams, and diagnoses.
• Manage acute and chronic conditions following established protocols.
• Prescribe medications and treatments as appropriate.
• Conduct monthly stability visits for long-term residents.
• Evaluate residents with changes in condition.
• Collaborate with interdisciplinary teams and attending physicians.
• Complete accurate documentation and CPT coding.
• Facilitate referrals to specialists and other healthcare providers.
