Telos Health Systems in Bedford, KY seeks an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide geriatric care in Skilled Nursing and Assisted Living settings.

As a SNF-ist, you will lead internal medicine care, collaborate with clinicians, perform comprehensive assessments, prescribe treatments, and ensure quality patient outcomes. Qualifications

• PMHNP certification and active license are required.
• DEA registration must be active.
• Minimum one year experience in Skilled Nursing preferred.
