Telos Health Systems seeks a Psychotherapist to provide mental health evaluations and psychotherapy in Skilled Nursing and Assisted Living facilities. Role offers clinical autonomy, strong administrative support, and a flexible schedule while delivering care to geriatric residents.

Qualifications

• Active Pennsylvania license as LCSW, LPC, LMFT, or licensed psychologist.
• Master's degree or doctoral degree in a mental health field.
• Ability to work independently while collaborating with a clinical team.
• Strong communication and clinical documentation skills.
• Geriatric population experience preferred.
• Experience in Skilled Nursing/Assisted Living or long-term care preferred.

Responsabilités

• Conduct mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update treatment plans with the care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to federal, state, and organizational standards.
