Job summary
Telos Health Systems, Inc. is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to serve as the SNF-ist, providing internal medicine services for geriatric residents in skilled nursing or assisted living settings.
The role includes day-to-day leadership within the facility’s care team and collaboration with physicians and staff. The position offers a $5,000 sign-on bonus and a flexible schedule with two nonconsecutive daytime weekdays.
Qualifications
- Certification as Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active license and DEA #
- Minimum one year experience in Skilled Nursing or equivalent setting
- Ability to thrive in a fast-paced healthcare environment
Responsibilities
- Lead internal medicine services as SNF-ist and clinical leader with interdisciplinary team
- Oversee day-to-day care for short and long-term residents
- Provide health assessments, differential diagnosis, treatment plans under protocol
- Prescribe medications and manage chronic illnesses
- Perform comprehensive physical examinations and document CPT coding
- Develop patient management policies and coordinate referrals
