Job summary
Telos Health Systems, Inc. is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) in Cookeville, TN. Join a clinician-first team providing geriatric care in Skilled Nursing/Assisted Living settings.
You will lead medical care, coordinate with a multidisciplinary team, and perform comprehensive assessments and treatment plans for residents. The role offers a flexible schedule of two nonconsecutive weekdays, daytime hours, W2 compensation, and strong professional development support.
Qualifications
- Certification as a PMHNP and active license with DEA number.
- Minimum one year of experience in a Skilled Nursing setting (preferred).
- Strong communication and leadership abilities.
- Ability to thrive in a fast-paced healthcare environment.
Responsibilities
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments per protocol.
- Lead the SNF team and oversee resident care with nursing staff and medical directors.
- Perform physical examinations and coordinate referrals to specialists.
- Document thoroughly and manage CPT coding.
- Conduct monthly stability visits and evaluate changes in condition.
- Support advanced care planning.
