Telos Health Systems invites a Psychiatric Mental Health Nurse Practitioner to join our Bedford, KY team. You will lead geriatric care in Skilled Nursing and Assisted Living settings, performing comprehensive assessments, diagnoses, and treatments in coordination with a facility-wide medical team.

The role emphasizes strong communication, teamwork, and clinical leadership, with a 1-day-per-week schedule chosen from Monday to Friday.

Formación

• Must be a Licensed PMHNP with current certification.
• Active state license and valid DEA registration required.
• Minimum of 1 year in Skilled Nursing/long-term care preferred.
• Excellent written and verbal communication skills; ability to lead teams.

Responsabilidades

• Provide PMHNP-level assessment, diagnosis, and treatment in SNF/AL settings.
• Collaborate with interdisciplinary team and physicians; guide daily patient care.
• Prescribe medications and manage chronic conditions per facility protocols.
• Perform regular visits and advanced care planning; document with CPT coding.
