Job summary

Telos Health Systems seeks an experienced Advanced Practice Provider (NP or PA) to be the in-house SNF medical provider in Cookeville, TN. You’ll deliver internal medicine care to geriatric residents in SNFs and ALFs with autonomy and teamwork to improve long-term care outcomes.

Qualifications

• Active, unencumbered APRN/APN or PA-C license in Kentucky.
• Current DEA registration.
• Master’s degree from an accredited NP/PA program.

Responsibilities

• Provide internal medicine care for SNF/ALF residents.
• Perform comprehensive health assessments and exams.
• Prescribe medications and treatments as appropriate.
• Coordinate with interdisciplinary teams and attending physicians.
