Job summary

Telos Health Systems, Inc. in Roanoke, VA is seeking a part-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide geriatric care in Skilled Nursing and/or Assisted Living settings.

The role offers a 1-2 day schedule with a compensation model that can be salary or capitated via wRVU, with a target of 75 visits per week and a $10,000 sign-on bonus. You will collaborate with an interdisciplinary team, perform comprehensive assessments, manage medications, and participate in care

Qualifications

• PMHNP certification and valid license, DEA registration.
• Minimum one year in Skilled Nursing or SNF setting preferred.
• Master’s degree in Nursing or related field.

Responsibilities

• Provide comprehensive health assessments and differential diagnoses.
• Prescribe treatments under the facility protocol.
• Perform comprehensive physical examinations.
• Develop and implement patient management policies and procedures.
• Facilitate referrals and communicate with attending physicians.
• Document care and CPT coding.
• Monthly stability visits and evaluations for changes in condition.
• Advanced Care Planning.
