Job summary

Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner, APRN, or Physician Assistant to provide PM&R/Physiatry services at designated SNFs or post-acute rehab centers in Lake City, FL.

You will collaborate with interdisciplinary teams, lead clinical care, and help manage conditions common in SNFs while enjoying a balanced schedule and strong benefits.

CRC offers autonomy, training, education reimbursements, a 40-hour work week, and a comprehensive benefits package.

Qualifications

• Post-acute/SNF experience preferred.
• NP/PA licensure in Florida required.
• Willingness to work at SNF/post-acute sites with travel as needed.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Lead clinical care and collaborate with the interdisciplinary team.
• Coordinate with PCPs and therapy teams to manage common SNF conditions such as CVA and postoperative care.
• Monitor patient progress and adjust plans with PT/OT/SLP teams.
• Attend multidisciplinary meetings and promote continuity of care.
