Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner/APRN clinician to provide PM&R/Physiatry services at designated SNFs or post-acute rehab centers in Allentown, PA. The role emphasizes autonomy, collaboration with inter-disciplinary teams, and improving patient independence.

The ideal candidate holds PA licensure, post-acute space experience is preferred, and you will work a standard 8-hour day with a 5-day work week in a full-time W-2 role with competitive pay and benefits.

Qualifications

• Nurse Practitioner/APRN licensed in PA.
• Ability to work legally in the U.S.
• Post-acute space experience preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with strong clinical leadership.
• Collaborate with PCP teams for SNF/post-acute care management.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow patients to track progress in therapy.
• Assess barriers to therapy and manage pain or neurogenic issues.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education.
