Job summary

Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner, APRN, or Physician Assistant to provide PM&R services in a designated SNF/post-acute rehab center in Lake City, FL. You will work with the post-acute therapy team to optimize rehabilitative outcomes.

This full-time, 40-hour W-2 role offers a stable schedule (8 a.m.–5 p.m., Monday–Friday), generous benefits, malpractice coverage, CEU opportunities, and a supportive culture focused on patient independence and high-quality care.

Qualifications

• Nurse Practitioner/APRN/ Physician Assistant licensed in Florida.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.
• 40 hours per week.
• PM&R/Physiatry background desired.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in a designated SNF/post-acute rehab center.
• Represent CRC with strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams for management of SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
