Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R Clinician (NP/PA) to provide specialized rehabilitation services at designated SNF or post-acute centers in Worcester/Southbridge, MA. You will collaborate with the therapy and medical teams to optimize patient outcomes and promote independence.

The ideal candidate will have a MA license and NP/PA credentials, post-acute experience preferred, and a patient-centered approach with strong communication and leadership abilities.

Qualifications

• Nurse Practitioner/APRN/PA license is required.
• Massachusetts license is required or eligible to practice in MA.
• Ability to work in the U.S. without sponsorship.
• Post-acute care experience is preferred.
• Demonstrated empathy and a patient-centered approach.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Represent CRC in the facility with clinical leadership and interdisciplinary teamwork.
• Collaborate with PCP teams to manage common SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
