Comprehensive Rehab Consultants in Hampton, VA seeks a PM&R clinician (NP/PA) to provide rehab services in a SNF/post-acute setting. You will collaborate with PT/OT/SLP and PCP teams, lead clinical care, and advance patient independence.

This 40-hour, W-2 role offers a competitive salary with generous benefits, plus flexible hours around 9 AM–4 PM, Monday–Friday scheduling and opportunities for professional growth.

Qualifications

• SNF or post-acute experience preferred.
• Licensed as NP/PA/APRN in Virginia.
• Ability to work autonomously and with compassion.

Responsibilities

• Provide PM&R/physiatry services at a designated SNF/post-acute facility.
• Collaborate with inter-disciplinary team and CRC to coordinate care.
• Work with PCPs to manage common post-acute conditions.
• Support therapy teams to improve patient function.
• Attend meetings and contribute to culture-building initiatives.
