Comprehensive Rehab Consultants seeks a PM&R clinician (NP/PA) to deliver rehab services in SNF settings and post-acute centers. You will work with an interdisciplinary team to optimize patient independence and outcomes, with flexible hours around a 9 AM–4 PM schedule.

Qualifications

• Clinicians must be able to work autonomously within SNF teams.
• Experience in post-acute space is preferred.
• Strong interpersonal skills and compassion for geriatric patients.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF settings.
• Collaborate with interdisciplinary care teams and PCPs.
• Support patient function and quality of life with therapy teams.
• Attend meetings and help bridge therapy and internal medicine.
