Job summary

Comprehensive Rehab Consultants (CRC) seeks a PM&R clinician (NP/PA) in Cincinnati/Lebanon, OH, for a 32-hour W-2 role. You’ll deliver specialized PM&R services within SNF/post-acute settings and collaborate with an interdisciplinary team to optimize patient outcomes.

Ideal candidates will have post-acute experience, Ohio licensure, and the ability to function autonomously while driving clinical excellence and education within the facility.

Qualifications

• Must hold NP/APRN/PA license and be licensed in Ohio.
• Post-acute care/post-acute rehabilitation experience preferred.
• Strong clinical leadership and collaboration with inter-disciplinary teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF or post-acute center.
• Represent CRC in facility with strong clinical leadership and collaboration with care teams.
• Collaborate with PCPs to manage conditions common in SNFs (CVA, neurological conditions, postoperative surgeries).
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
