Job summary

Comprehensive Rehab Consultants (CRC) is seeking a Nurse Practitioner/Physician Assistant to provide PM&R/Physiatry services at a designated SNF or post-acute rehab center in Cincinnati, OH. The clinician will lead multidisciplinary teams and collaborate with PCPs to manage conditions common in post-acute care, including stroke and progressive neurological disorders.

Qualifications

• NP/APRn/PA license in Ohio.
• Ability to work in the U.S.
• Post-acute space experience preferred.
• Great attitude and desire to deliver the best patient care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage SNF-related conditions such as CVA and progressive neurological issues.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture-building and ongoing education including Didactic lectures and regional meetings.
