Job summary

Comprehensive Rehab Consultants (CRC) seeks a Part-time NP/PA/PM&R Clinician to deliver Physiatry services in a designated SNF or post-acute rehab setting in Springfield and Dayton, OH. You will work with the post-acute therapy team to optimize rehab outcomes and promote patient independence.

The role offers flexible daytime hours around 9 AM–4 PM, 401(k) with 2–3% contribution, full malpractice coverage, access to CEUs, and a laptop, with opportunities for growth and leadership as CRC expands.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Active Ohio license or eligible to practice in the U.S.
• Post-acute or SNF experience preferred.
• Must be able to work in the U.S.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Collaborate with interdisciplinary care team and facility staff.
• Support management of conditions common in SNFs and track patient progress.
• Attend multidisciplinary meetings and contribute to culture building.
