Job summary
Comprehensive Rehab Consultants (CRC) in Cincinnati/Lebanon, OH is hiring an NP/PA PM&R Clinician to deliver Physiatry services at a designated SNF or post-acute rehab center. You will collaborate with the post-acute therapy team to optimize rehabilitation outcomes.

Qualifications
- Nurse Practitioner/APRN/PA license
- Ohio license
- Ability to work in the U.S.
- Post-acute space experience preferred
- Desire to deliver high-quality patient care
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant
- Represent CRC in your facility with clinical leadership
- Collaborate with PCP teams for SNF/post-acute conditions
- Work with PT/OT/SLP to improve patient function
- Follow therapy patients to track progress
- Address barriers to therapy (pain, spasticity, neurogenic issues)
- Attend multidisciplinary meetings and bridge therapy with internal medicine
- Engage in culture building and ongoing education
