Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA/Physician Assistant) to serve as a rehab consultant at designated SNFs or post-acute rehab centers in Maine. You will lead clinical care, collaborate with interdisciplinary teams, and help optimize rehabilitative outcomes for geriatric patients.

The role offers autonomy with a flexible schedule around 9 AM–4 PM, comprehensive benefits, and opportunities for professional growth within CRC's training programs.

Competenze

• PM&R/Physiatry knowledge and clinical skills in post-acute settings.
• Autonomy in managing clinical workflow and decision-making.
• Strong collaboration with interdisciplinary care teams.

Mansioni

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute centers.
• Represent CRC with strong clinical leadership within the facility team.
• Collaborate with PCP teams to manage common SNF conditions (CVA, neurodegenerative diseases, postoperative ortho).
• Coordinate with PT/OT/SLP to improve patient function and quality of life.
• Monitor patient progress and follow therapy plans across teams.
• Identify and address barriers to therapy, incl. pain, spasticity, neurogenic issues.
• Participate in multidisciplinary meetings and bridge therapy with internal medicine.
• Support culture building and ongoing education through lectures and regional meetings.
