Job summary

Comprehensive Rehab Consultants seeks a PM&R clinician (NP/PA) to provide specialized Physiatry services at a designated SNF/post-acute center in Hampton, VA. You will collaborate with the therapy and medical teams to optimize rehabilitation outcomes for geriatric patients.

The role is 40 hours weekly, W-2, with a competitive salary and comprehensive benefits. VA license and NP/PA certification are required, with post-acute experience preferred.

Qualifications

• Nurse Practitioner, APRN, or PA license is required.
• Virginia license is required.
• Experience in post-acute space preferred.
• Able to work in the U.S.
• 40 hours/week full-time schedule.

Responsibilities

• Provide PM&R services as a rehab consultant in SNF/post-acute settings.
• Lead and collaborate with interdisciplinary care teams.
• Coordinate with PCPs for management of common SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Monitor patient progress and barriers to therapy; participate in meetings.
