Job summary

Comprehensive Rehab Consultants is seeking a Nurse Practitioner/PA with PM&R to deliver specialized care at designated SNFs in the Virginia Beach area. You will lead rehab teams, coordinate with PCPs, and focus on improving function for geriatric patients.

Qualifications

• Nurse Practitioner/APRN/ PA qualification required.
• Virginia license to practice in the U.S. is required.
• Experience in post-acute space is preferred.
• Strong patient-care attitude and ability to deliver high-quality care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility with clinical leadership.
• Collaborate with PCP teams for SNF/post-acute conditions.
• Work with PT/OT/SLP to improve patient function.
• Follow therapy patients to track progress and outcomes.
• Bridge therapy and internal medicine through multidisciplinary meetings.
• Engage in culture-building and ongoing education.
