Comprehensive Rehab Consultants is seeking a PM&R Clinician to provide specialized PM&R services at a designated SNF/post-acute rehab center in Pleasanton, CA. The role emphasizes autonomous practice within a multidisciplinary team to improve patient independence and outcomes.

The ideal candidate will hold a NP/PA license in California with post-acute/SNF experience, collaborate with PCPs, and participate in education and leadership opportunities within CRC.

Qualifications

• Nurse Practitioner/APRN/PA license valid in California.
• California license required and ability to work in the U.S.
• Post-acute/SNF experience preferred.
• Strong autonomous practice capability.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to help manage SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education.
