Comprehensive Rehab Consultants (CRC) invites a NP/PA PM&R Clinician to join our post-acute care team in Aurora, CO. You will provide specialized PM&R/Physiatry services at a designated SNF or rehab center, collaborating with the interdisciplinary team to improve function and independence.

Qualifications

• Active NP/PA license in Colorado.
• Post-acute/post-SNF experience preferred.
• Ability to work autonomously within a clinical team.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCPs and therapy teams to manage common SNF conditions.
• Follow therapy patients to track progress and outcomes.
• Attend multidisciplinary meetings and bridge therapy to internal medicine.
