Job summary

Comprehensive Rehab Consultants (CRC) seeks a PM&R clinician to serve SNF/post-acute patients, delivering specialty rehabilitation services in a designated facility. You will work with an interdisciplinary team to optimize outcomes, promote independence, and contribute to a patient-centered care model.

The role offers autonomy, a collaborative culture, and professional development through CRC training and certifications, with a typical daytime schedule and a competitive salary.

Qualifications

Experience in post-acute care settings, preferably SNF.
Ability to function autonomously in a clinical setting.
Strong interpersonal skills and empathy for geriatric patients.
Commitment to CRC culture and continuous learning.

Responsibilities

Provide PM&R/Physiatry services as a rehab consultant.
Represent CRC with clinical leadership in the facility.
Collaborate with PCPs on SNF conditions (CVA, neurological, post-op).
Coordinate with PT/OT/SLP to improve function and QoL.
Monitor patient progress and address barriers to therapy.
Attend multidisciplinary meetings and support clinical leadership.
