Job summary
Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner/Physician Assistant with PM&R expertise to deliver rehabilitative medical care in designated SNFs or post-acute centers in Connecticut. You will lead, collaborate with therapy and primary care teams, and drive patient independence while maintaining a balanced schedule.
This full-time role offers a starting salary of $90,000, a 8-hour day between 8 AM and 5 PM, and a supportive culture with professional development, a virtual
Qualifications
- Nurse Practitioner/APRN/PA license and CT license required.
- Post-acute care experience preferred.
- Ability to work autonomously.
- Strong interpersonal skills and empathy.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC with the facility and collaborate with the interdisciplinary care team.
- Collaborate with PCPs to manage SNF conditions like CVA and post-op orthopedic surgeries.
- Work with PT/OT/SLP teams to improve patient function and quality of life.
- Attend multidisciplinary meetings and bridge therapy with internal medicine.
- Engage in culture-building opportunities and ongoing education.
