Job summary

Comprehensive Rehab Consultants seeks a PM&R Clinician (NP/PA) to support SNF/post-acute care in Hobart, IN. This 24-hour, part-time W2 role offers a stable daytime schedule (around 9 AM–4 PM) with malpractice coverage and generous CEU opportunities. A remote virtual scribe and tech tools reduce documentation burden while you lead clinical care and collaborate with the care team to improve patient outcomes and quality of life.

Qualifications

• NP/APRN/PA License
• Indiana License
• Experience in post-acute space preferred
• Ability to work in the U.S.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCP teams on management of SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Track patient progress and follow therapy outcomes.
• Engage in multidisciplinary meetings and bridge therapy with internal medicine.
• Participate in culture-building and ongoing education.
