Job summary
Comprehensive Rehab Consultants in Antioch, TN is seeking a part-time PM&R Clinician (Nurse Practitioner/PA) to join our post-acute care team at a designated SNF. You will provide PM&R/Physiatry services and work with the interdisciplinary team to optimize patient function, while maintaining a balanced schedule around 9 AM–4 PM.

Qualifications
- Nurse Practitioner/APRN/PA license required.
- Tennessee license.
- Ability to work in the U.S.
- Post-acute experience preferred.
- Great attitude and desire to deliver the best patient care.
Responsibilities
- Provide PM&R services as a rehab consultant.
- Represent CRC in your facility through clinical leadership.
- Collaborate with PCP teams to manage SNF conditions such as CVA and postoperative surgeries.
- Work with PT/OT/SLP teams to improve patient function and quality of life.
- Follow therapy patients to track progress.
- Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
- Attend multidisciplinary meetings and bridge therapy and internal medicine.
- Engage in culture building and ongoing education.
