Comprehensive Rehab Consultants seeks a PM&R clinician (Nurse Practitioner/PA) to deliver specialized physiatry services in SNF/post-acute centers in Virginia. You will work with an interdisciplinary team to optimize patient independence and functional outcomes.

Ideal candidates hold a Virginia NP/PA license, have post-acute experience, and can autonomously manage a 40-hour workweek with emphasis on high-quality patient care and collaboration with facility partners.

Qualifications

• Nurse Practitioner/APRN/ PA with Virginia license.
• Post-acute or SNF/post-acute care experience preferred.
• 40 hours per week schedule and full-time status.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Collaborate with facility staff and interdisciplinary care team.
• Manage conditions common in SNFs (e.g., CVA, neurological and postoperative cases).
• Coordinate with PT/OT/SLP to improve function and quality of life.
• Attend multidisciplinary meetings and support culture-building initiatives.
