Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R clinician (NP/PA) to provide rehabilitation services at designated SNF/post-acute centers in Colonial Heights and Hopewell, VA. You’ll collaborate with therapy and medical teams to optimize patient independence and outcomes.

The role offers autonomy, a flexible schedule around 9 AM–4 PM, and opportunities for continued education, leadership development, and professional advancement within the CRC network.

Qualifications

Nurse Practitioner/APRN/PA License
Virginia License
Experience in post-acute space preferred
A great attitude and desire to deliver the best patient care

Responsibilities

Provide PM&R/Physiatry services as a rehab consultant
Represent CRC in your facility with clinical leadership
Collaborate with PCP teams for SNF/post-acute conditions
Work with PT/OT/SLP teams to improve function and quality of life
Follow therapy patients to track progress
Assess and manage barriers to therapy (e.g., pain, spasticity)
Attend multidisciplinary meetings and bridge therapy and internal medicine
Engage in culture building and ongoing education
