• NP/PA licensed in Ohio; post-acute care experience preferred.
• Ability to work autonomously in SNF/post-acute settings.
• Strong collaboration with interdisciplinary teams and PCPs.

Responsibilities

• Provide PM&R/Physiatry services in SNF/post-acute centers.
• Lead and represent CRC within the facility care team.
• Collaborate with PT/OT/SLP to improve patient function and outcomes.
• Monitor patient progress and coordinate with therapy teams.
