Job summary

Comprehensive Rehab Consultants (CRC) is seeking a part-time NP/PA PM&R Clinician based in LaGrange, GA. The role focuses on PM&R services exclusively at a designated SNF or post-acute rehab center, collaborating with the rehabilitation team to optimize patient outcomes.

You will work with PCPs and therapy colleagues to manage common SNF conditions, maintain high-quality care, and promote patient independence, with flexible hours around a 9 AM–4 PM window.

Qualifications

• Nurse Practitioner, APRN, or PA license required.
• GA state license required.
• Post-acute/SNF experience preferred.
• Ability to work in the U.S.

Responsibilities

• Provide PM&R services as a rehab consultant.
• Lead in SNF facility with interdisciplinary team.
• Collaborate with PCP, PT, OT, SLP for patient care.
• Monitor patient progress and therapy barriers.
• Attend multidisciplinary meetings and promote culture.
• Support documentation and clinical leadership.
