Job summary

Comprehensive Rehab Consultants (CRC) seeks an NP/PA PM&R Clinician to deliver PM&R services at a designated SNF/post-acute rehab setting in LaGrange, GA. This is a 16-hour, part-time W2 role with hours around 9 AM–4 PM and weekday coverage.

You will work closely with the post-acute therapy team, PCPs, and SNF staff to improve function and patient outcomes, with opportunities for professional growth and leadership development within CRC.

Qualifications

• Post-acute space experience preferred.
• Ability to function autonomously.
• Strong interpersonal and communication skills.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant at the SNF/post-acute center.
• Represent CRC in the facility with clinical leadership and collaboration with the care team.
• Collaborate with PCPs to manage SNF-related conditions and work with PT/OT/SLP teams to improve function.
• Follow therapy patients to track progress and address barriers to therapy (pain, spasticity, neurogenic issues).
• Attend multidisciplinary meetings and contribute to culture building and education within CRC.
