Zusammenfassung

Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner/PA/PM&R clinician to provide specialized PM&R services at designated SNFs and post-acute centers in Buchanan, GA and Carrollton, GA. You will lead clinically and collaborate with rehab teams to optimize patient independence in geriatric post-acute care.

Qualifikationen

• Post-acute/post-acute care experience preferred.
• Ability to work autonomously.
• Strong interpersonal skills and the ability to understand the needs of facility partners and patients.

Aufgaben

• Provide PM&R/Physiatry services as rehab consultant.
• Represent CRC through clinical leadership in facility.
• Collaborate with PCP teams on SNF conditions.
• Work with PT/OT/SLP to improve function.
• Follow therapy patients to track progress.
• Assess/manage barriers to therapy (pain, neuro issues).
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education.
