Job summary

Comprehensive Rehab Consultants (CRC) seeks a PM&R clinician (NP/PA) to provide specialized PM&R services at a designated SNF or post-acute rehab center in Sarasota and St. Petersburg, Florida. You will work with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence.

This 40-hour, full-time W-2 role offers a competitive salary, comprehensive benefits, flexible hours around 9 AM–4 PM, and opportunities for ongoing education and leadership within CRC.

Responsibilities

• Provide PM&R services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage SNF conditions (CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess and manage barriers to therapy including pain, spasticity, neurogenic bowel/bladder.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
• Engage in culture-building opportunities and ongoing education.
