Job summary

Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA) to provide specialized Physiatry services at designated SNFs/post-acute centers in North Carolina. You will collaborate with an interdisciplinary team to optimize rehabilitative outcomes and promote patient independence.

The ideal candidate will have a background in post-acute care, strong autonomy, and a commitment to high-quality patient care within a physician-owned, growth-oriented organization.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• North Carolina license required.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage SNF patients' conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess and address barriers to therapy including pain, spasticity, neurogenic issues.
