Job summary

Comprehensive Rehab Consultants in Texas seeks a PM&R Clinician (NP/PA) to provide specialized PM&R services at designated SNFs/post-acute centers. You will lead clinically within the facility, collaborating with PCPs and therapy teams to optimize patient independence in a fast-growing, physician-owned group.

The role offers a 32-hour W-2 schedule in Nacogdoches/Lufkin, TX, with 8-hour days (8 AM–5 PM), a 4-day work week, and comprehensive benefits including malpractice coverage and education

Qualifications

• Nurse Practitioner/APRN/PA license is required or eligible,
• Texas license is required to practice in the state
• Experience in post-acute care space is preferred

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility through clinical leadership with the care team.
• Collaborate with PCP teams for SNF/post-acute conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine
