Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R clinician (NP/PA) in Texas City, TX to serve at a designated SNF or post-acute rehab center. The clinician will partner with the post-acute therapy team to optimize rehabilitative outcomes and patient independence.

The ideal candidate will be licensed as an NP/APR N/PA in Texas, with SNF/post-acute experience preferred, and a demonstrated ability to operate autonomously within a collaborative interdisciplinary team.

Qualifications

• NP/APRN/PA license in Texas is required.
• Experience in post-acute space is preferred.
• Ability to function autonomously within a SNF/post-acute setting.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility through clinical leadership and collaboration with care team.
• Collaborate with PCP teams to manage conditions common in SNFs.
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress and manage barriers to therapy.
• Attend multidisciplinary meetings and promote culture within the SNF team.
