Job summary

Comprehensive Rehab Consultants (CRC) is seeking a Part-time NP/PA/PM&R clinician to join our post-acute rehabilitation team in Bloomfield, NM. You will deliver PM&R/physiatry services at designated SNFs, collaborating with interdisciplinary teams to optimize patient independence and function.

Qualifications

• Post-acute care experience is preferred.
• Ability to work autonomously within a team.
• Strong communication and patient-centered approach.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant at SNF/post-acute facilities.
• Collaborate with facility teams and PCPs to manage common post-acute conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Monitor patient progress and assist with documentation and care coordination.
• Participate in multidisciplinary meetings and continuous education.
