Job summary
Comprehensive Rehab Consultants in Modesto, CA is seeking a PM&R clinician (NP/PA) to provide specialized rehab services at SNFs and post-acute centers. You will lead clinical care with the interdisciplinary team, emphasize geriatric function, and promote patient independence in a supportive, physician-owned practice.

Qualifications
- Nurse Practitioner/APR/PA license (CA).
- Licensed to practice in the United States.
- Post-acute/SNF experience preferred.
- Ability to work independently.
- Compassionate, patient-centered care.
Responsibilities
- Provide PM&R/Physiatry services as rehab consultant.
- Represent CRC in facility with strong clinical leadership.
- Collaborate with PCP teams for SNF conditions.
- Coordinate with PT/OT/SLP to improve function and independence.
- Attend multidisciplinary meetings and bridge therapy with internal medicine.
- Support documentation and workflow improvements for the team.
- Engage in culture building and regional education activities.
