Job summary

Comprehensive Rehab Consultants seeks a Nurse Practitioner/PA in PM&R to provide specialized rehabilitation services at a designated SNF or post-acute center in San Diego, CA. The clinician will work with the therapy team and PCPs to optimize outcomes, promote independence, and lead clinical care within CRC.

The role offers full-time hours (around 9 AM – 4 PM), a competitive salary starting at $150,000 per year, and comprehensive benefits including health insurance, generous PTO, CEU

Qualifications

• Autonomy in clinical practice and decision-making.
• Strong collaboration with PCPs and therapy teams.
• Geriatric post-acute care experience preferred.

Responsibilities

• Provide PM&R/Physiatry services in SNF/post-acute settings.
• Lead and represent CRC within facilities and care teams.
• Collaborate with PCP, PT, OT, and SLP teams to improve patient function.
• Monitor patient progress and barriers to therapy.
• Attend multidisciplinary meetings and support culture building.
