Job summary
Comprehensive Rehab Consultants seeks a Nurse Practitioner/Physician Assistant to provide PM&R/Physiatry services in designated SNFs or post-acute rehab centers in Rockville, Cromwell, and Middletown, CT. The role emphasizes autonomy, collaboration with interdisciplinary teams, and improving patient independence.
The ideal candidate is experienced in post-acute care, licensed in Connecticut, and committed to continuous improvement and clinical leadership.
Qualifications
- The Ideal CRC Clinician will have a passion for post-acute healthcare and serving the geriatric population.
- SNF or post-acute experience preferred.
- Autonomy, adaptability, and growth mindset are valued.
Responsibilities
- Provide specialized PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility with clinical leadership and collaboration with the care team.
- Collaborate with PCP teams to manage conditions common in SNFs (CVA, progressive neuro conditions, post-op orthopedics).
- Work with PT/OT/SLP to improve patient function and quality of life.
- Monitor patient progress and barriers to therapy; participate in multidisciplinary meetings.
