Job summary

Omni Family of Services Tennessee Inc. seeks a Placement Coordinator to arrange foster youth placements into licensed treatment foster homes. You will review referrals, gather data, and coordinate with regional leadership to meet placement needs.

The role requires a bachelor’s degree with experience in child welfare and a valid driver’s license, with a focus on safety, HIPAA and CMS guidelines while supporting families and clients.

Qualifications

• Bachelor’s degree with a minimum of one year experience in the child welfare system or equivalent education/experience.
• Valid Driver’s License with proof of automobile insurance and reliable transportation.
• Commitment to safety, policy compliance (OSHA, CMS, HIPAA) and regulatory standards.

Responsibilities

• Place foster care youth referred by the department into licensed treatment foster homes.
• Review information from referring sources and gather additional data to support placement decisions.

As the Placement Coordinator you would be responsible for the placement of foster care youth referred by the department who are in need of placement in a licensed treatment foster home. The Child Placement Coordinator will review all information provided by the referring source and collect any additional information that would help in making the best placement. In addition, they will coordinate with their Regional Director, Team Leaders and RC’s on placement needs within their region. The Child Placement Coordinator is expected to perform all job description responsibilities in alignment with the mission and core values of Omni Family of Services and comply with all Omni Family of Services policies and procedures, OSHA, CMS, HIPAA, local, state and federal regulations.
