Job summary

Omni Family of Services Tennessee Inc in Cleveland, TN seeks a Care Coordinator to work with clients and families in home settings, delivering trauma-informed, client-centered care and coordinating services.

Qualifications

• Bachelor's degree in behavioral health field (counseling, social work or psychology preferred).
• Valid Driver’s License and proof of automobile insurance.
• Certification in crisis prevention initiatives and universal precautions required.

Responsibilities

• Meet with client(s) and family in the client’s home or residence for regularly scheduled care.
• Phone call triage assessments during off months/weeks of face-to-face care.
• Meet with extended family and community supports at locations such as school, court or job site as needed.
• Hours generally do not exceed 40 per week; on-call and after-hours responsibilities.

The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face to face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week. On-call and after-hours
