Job summary

Omni Family of Services Tennessee Inc is seeking a Care Coordinator to meet with clients and families in homes, coordinating services across community settings. The role supports safety, healing, and advocacy through trauma-informed practice and collaboration with extended networks.

The position emphasizes flexible scheduling, on-call availability, and a commitment to empowering families on their journey to stability and growth.

Qualifications

  • Bachelor’s degree in behavioral health field required (counseling, social work, or psychology preferred).
  • Valid Driver’s License and proof of automobile insurance required.
  • Crisis prevention certification and universal precautions required.

Responsibilities

  • Meet with clients and families in home or residence for regular assessments and support.
  • Coordinate services across foster/adoptive/biological family, and link with community supports as needed.
  • Engage with schools, courts, and workplace locations to meet service goals.
  • Maintain caseload and schedule within a 40-hour work week; be available for on-call after hours.

Role

The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face to face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week. On-call and after-hours
