Job summary

Omni Family of Services Tennessee Inc. seeks a Care Coordinator to meet with clients and families in home settings, coordinating care and advocacy across school, court, and community locations. Hours vary but typically stay within 40 hours per week, with on-call and after-hours coverage.

The role emphasizes safety, trauma-informed care, and collaboration with multidisciplinary teams to support children, adults, and families.

Qualifications

  • Bachelor's degree in a behavioral health related field such as counseling, social work, or psychology preferred.
  • Ability to work with clients and families in home and community settings.
  • Valid driver's license and reliable transportation may be required.

Responsibilities

  • Meet with client and family in home or residence for assessments and support.
  • Coordinate care with client, family, and community supports across settings.
  • Schedule regular visits and on-call coverage to meet client needs.
  • Provide crisis prevention guidance and connect to services as needed.

The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face to face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week.   • A minimum of a bachelor’s degree required, with said degree being in a behavioral health related field such as counseling, social work, or psychology preferred.
  • Valid Driver’s License, Proof of Automobile Insurance and Reliable Transportation
  • Certification in proper crisis prevention initiatives and universal precautions required.
