Description:Function independently to provide high-quality patient care in a collaborative and patient-centric manner.Care may occur in various settings and through different modalities, including direct patient interaction in hospital and/or clinic, as well as via telephone, video, or secure email when appropriate.Gather patient history, perform physical exams, review prior records, and evaluate ancillary data using sound judgment and critical problem-solving skills.Develop and monitor appropriate treatment plans, communicating effectively with patients and families when applicable.Provide efficient and compassionate care for both patients and their families through a multi-disciplinary approach that emphasizes communication, continuity, coordination, and evaluation.Offer relevant education and counseling to patients and families to support care plans, treatment, and discharge.Collaborate with healthcare team members, leadership, and medical staff to ensure safe patient care.Communicate pertinent findings and information to physicians and the care team.Assist collaborating physicians with managing patients in accordance with care site policies and approved Scope of Practice.Ensure compliance with regulatory standards and practice metrics.Document in accordance with regulatory and care site guidelines.Promote mission, vision, and values.Perform other duties as assigned.May be required to float to other departments based on business needs.May need to be placed on-call during a regularly scheduled shift.
