Job summary

Chestnut Health in Illinois seeks a Physician Assistant to join our physiatry team, delivering patient-centered rehabilitation care in SNF settings. You will collaborate with therapists, nurses, and physicians to develop comprehensive planning and support functional independence throughout the care continuum.

Qualifications

• Licensed to practice in the state of employment
• Current Controlled Substance License (required on start date)
• Current DEA License (required on start date)
• NCCPA Certification
• Interest in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic experience (nice to have)
• Physiatry experience (nice to have)
• Skilled Nursing experience (nice to have)

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
• Conduct patient assessments including history, exams, and diagnostics to determine rehab goals.
• Monitor progress and adjust treatment plans to improve functional outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Educate patients and families about rehabilitation processes, outcomes, and post-discharge care.
