Job summary

Chestnut Health in Belleville, IL, is seeking a Physician Assistant to join our physiatry team. You will collaborate with therapists, nurses, and physicians to develop comprehensive rehabilitation plans that restore function and independence for SNF patients.

Qualifications

• Licensed to practice in the state of employment.
• Current Controlled Substance License required by start date.
• Current DEA License required by start date.
• NCCPA certification obtained.
• Interest in Physiatry/PM&R.
• Experience in Skilled Nursing Facilities is a plus.

Responsibilities

• Collaborate with therapists, nurses, and physicians to develop and implement rehabilitation plans for diverse needs.
• Conduct patient assessments including history, exams, and evaluations to set rehabilitation goals.
• Monitor progress and adjust treatment plans to optimize function and independence.
• Participate in multidisciplinary meetings, case conferences, and ongoing education to enhance skills.
• Maintain accurate EMR documentation of evaluations, plans, progress, and discharges.
• Educate patients and families about the rehabilitation process and post-discharge care.
