Job summary

Chestnut Health in Benton, IL seeks a Physician Assistant to join our physiatry team. You will collaborate with therapists, nurses and physicians to develop comprehensive rehabilitation plans that restore function and independence for patients in SNFs.

Responsibilities include patient assessments, documenting evaluations and progress in EMR, and educating patients and families about the rehab process and post-discharge care.

Qualifications

• Licensed to practice as a Physician Assistant in Illinois.
• Current DEA and CDS licenses required on start date.
• NCCPA certification and active PA license.
• Interest or experience in Physiatry/Rehabilitation.
• SNF experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including histories, exams, and diagnostics, to determine rehab goals
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence
• Participate in multidisciplinary meetings, case conferences and educational activities
• Maintain accurate documentation in electronic medical records
• Provide education and counseling to patients and families about rehabilitation and post-discharge care
